Job Descriptions

REGISTERED NURSE

JOB SUMMARY:  Reports to supervisor.  Upon
order of the physician, dentist or podiatrist,
provides and directs nursing care services in the
home according to agency policies and
procedures.  Utilizes Agency and community
resources to fulfill individual patient goals
developed in coordination with the patient, family
and physician.

DUTIES:
  1. Makes the initial evaluation visit and any
    additional follow-up visits.
  2. Regularly re-evaluates the patient’s nursing
    needs.
  3. Initiates/provides those services requiring
    substantial and specialized nursing skills in
    accordance with the physician’s plan of
    care and necessary revisions.
  4. Initiates appropriate preventative and
    rehabilitative nursing procedures.
  5. Notifies the patient’s attending physician,
    dentist or podiatrist and other professional
    persons and responsible staff of significant
    changes in the patient’s condition
    according to agency policy.
  6. Assists in coordination of all care and
    services provided.
  7. Counsels/teaches the patient and family in
    meeting nursing, medical needs and
    procedures/treatments.
  8. Prepares documentation including clinical
    and progress notes according to agency
    policies.
  9. Participates in inservice programs and
    supervises and teaches other nursing
    personnel, as appropriate.
  10. Observes patient’s clinical condition
    including response to treatments, goals of
    care and changes in physical and emotional
    status, documenting as appropriate.
  11. Teaches and evaluates the patient and
    family’s ability to independently manage at
    home.
  12. Provides safe, quality, appropriate care
    according to physician orders, the
    applicable Nurse Practice Acts, professional
    competency and training, federal and state
    laws, and agency policy.
  13. Provides care following safety, infection
    control, universal precaution, hazardous
    materials and waste policies and procedures.

QUALIFICATIONS:
  1. Currently licensed to practice as a
    Registered Nurse by the State of California.
  2. One-year prior professional nursing
    experience.

PHYSICAL REQUIREMENTS:
  1. Must be able to see, hear and speak
    without limitation for documentation,
    reports and other written or telephone
    communications.  
  2. Must be able to sit for long periods and lift
    a minimum of ten pounds.

LICENSED VOCATIONAL NURSE

JOB SUMMARY: Reports to Agency
Registered Nursing Supervisor for specific
patient care. According to the plan of
treatment and orders of the physician,
dentist or podiatrist, provides nursing
services in the home according to agency
policies and procedures. Reports on the
patient’s physical, social and environmental
condition. Assists RN Supervisor as directed
within LVN scope of practice.  Relief shift
coverage as assigned in the home.

DUTIES:
  1. Provides those services requiring
    nursing skills in accordance with the
    physician’s plan of care and in
    accordance with the Standards of
    Practice.
  2. Notifies the patient’s attending
    physician, dentist or podiatrist and
    other professional persons and
    responsible staff of significant
    changes in the patient’s condition
    according to agency policy.
  3. Prepares documentation including
    clinical and progress notes according
    to agency policies.
  4. Participates in in-service programs
    and teaches other nursing personnel,
    as appropriate.
  5. Assists physicians and registered
    nurse in performing specialized
    procedures.
  6. Observes patient’s clinical condition
    including response to treatments,
    goals of care and changes in physical
    and emotional status, and reports
    findings to registered nurse, as
    appropriate.
  7. Educates and instructs the patient,
    patient’s family and appropriate
    others in appropriate self-care
    techniques and activities.
  8. Provides safe, quality, appropriate
    care according to physician orders,
    the applicable Nurse Practice Acts,
    professional competency and
    training, federal and state laws and
    agency policy.
  9. Provides care following safety,
    infection control, universal
    precautions, hazardous materials and
    waste policies and procedures.

QUALIFICATIONS:
  1. Currently licensed to practice by the
    California Board of Vocational Nurse
    and Psychiatric Technician Examiner.
  2. One year prior professional nursing
    experience.
